Collection of Information

We look at the number of hits the website receives and keep track of the domains from which this website is accessed. To determine what our users are interested in, we may also look at the frequency of search words used in connecting you to this website

The information to which we refer includes details of your medical condition, your personal details such as address and telephone number and the forms and questionnaires which you complete to assist in the assessment of your condition.

The reason that the information is gathered is mainly to help us provide the most comprehensive health care possible. The settings in which this information may be accumulated or released include the following:

1. Communication with others involved in your care.

  • Your referring doctor will receive a letter outlining the outcome of your consultation.
  • At some point specific therapists or specialists who are also involved directly in your care may benefit from this knowledge also.

2. Administration

  • For the efficient running of our practice (and by law) we need to maintain records of your contact details and of our billing for medical services. On rare occasions this information may be released to a collection agency if necessary.

3. Research and Education

  • The QGMG doctors are active in medical research and in the education of other doctors about Rehabilitation and Geriatric Medicine. Where medical research is involved, the details of your condition may be required for a specific research project. Information regarding your condition may be combined with the information gained from other patients with similar problems and this may then be analysed to assist in providing the best possible treatment. Medical information gathered in this way is anonymous – all information is de-identified before inclusion in any research data and you will not be able to be identified.
  • Also, certain conditions may offer valuable teaching opportunities for other doctors. Photographs may be taken of your x-rays and presented, for instance, at an educational meeting attended only by medical professionals. Again you will not be recognisable by the material presented. The main reason for medical teaching is to ensure that the highest quality care is made available to the patients of this practice.

How we handle emails

We will preserve the contents of any QGMG email message that you send us if we believe that we have a legal requirement to do so. Emails sent to or from QGMG are routinely monitored for quality control, systems administration and legal compliance purposes.

QGMG places a high level of importance and value on the security of all information associated with our clients. We have security measures in place to attempt to protect against the loss, misuse and alteration of personal information under our control.

Personal information is de-identified and/or destroyed by us when it is no longer needed by us or when the person involved asks to be removed entirely from any method of communication with QGMG.

QGMG retains the information provided to us by you including possibly your contact and credit card details to enable us to monitor and verify transactions and customer details and to retain adequate records for legal and accounting purposes. This information is held on secure servers in controlled facilities, with 24 hour security.

Information stored within our network and computer systems can only be accessed by those within the organisation with authority and computer network password sanctions.

As a result of this, while we aim to protect the personal information of all users, QGMG can not ensure or warrant the security of any information transmitted to it from any online product or service and therefore, the users do so at their own risk. Access to and Correction of Information

QGMG wishes to only maintain information about its clients, customers, tenant companies, past companies and contractors that is correct, accurate, timely, relevant and appropriate.

We are more than happy to allow you to access the information we have on record for you so long as that request for information access is in accordance with the National Privacy Principles.

If you notice that your information is not correct, we will correct that information only if we are satisfied that the information relates to you.

Complaints

We will comprehensively investigate any complaints made and attempt to resolve any breach that may have occurred in relation to the collection, use or destruction of personal information held by QGMG in accordance with the Commonwealth Privacy Legislation. If you are not satisfied with the way it is investigated or the outcome of the procedure then you may request an independent person to further investigate your complaint. This is usually the Commonwealth Privacy Officer.

In accordance with the National Privacy Principles we may transfer personal information we have collected about you to someone other than you in a foreign country only if this such transfer is permitted by the National Privacy Principles.

Your IP address (the address that indentifies your computer on the internet and is recognised immediately by our web server) will be logged by QGMG anytime you visit our website to read or download information.
