The process of rehabilitation may include:
- a full functional assessment
- an individually tailored program recognising individual goals
- treatment from a team of experienced professionals
- individual and group treatment
- assistance to be as independent as possible
- a co-ordinated discharge plan
Their rehabilitation services include both day and in-patient programs.
St Vincent’s Hospital also has a Geriatric Evaluation and Management Unit for both inpatient and outpatient assessment services. It is a specialised program of services provided by an interdisciplinary team of healthcare professionals.
The GEM Program promotes a wellness approach to establish a baseline comprehensive functional assessment, identify patient and family goals and concerns, develop a tailored plan for interventional opportunities, promote independence in a safe environment, and provide information regarding resources that will help the patient.
